POSITION SUMMARY: Responsible for coordinating and monitoring the process of Self-Determination. Act as the point person for communications and problem-solving issues related to Self-Determination with the Fiscal Intermediary, Compliance Department, primary clinicians, and consumers. Assure agency Procedures and Medicaid requirements are met during the Self-Determination process. Provide training and education about the Self-Determination process to BMHA, the community and consumers. Assist clinical teams with processing requests and orders for assistive technology and environmental modifications. Assist community-based clinical teams with monitoring and addressing issues related to coverage plans, loss of Medicaid, General Fund utilization.
